under the e-Health program, every citizen should be identified by a Unique Health ID & a Health card which is tagged to the Aadhaar card. All other cards- eg - ESIC Member no, Mother & Child program ID - should be tagged to Aadhaar. Hospitals / Doctors/ labs all to use the primary Aadhaar / Health ID data for Registration & to include this field in their patient Regn form. Create a Unique tracking mechanism for tracking the Health services utilized by the individual.

There should be a system in place where we should be able to track which persion is given which medication by which doctor at what facility. The patient should be able to view all his medical history by using his account. This data should be private and could be shared only with the concent of the person.

All hospitals and clinics shall maintain proper digital system of providing the services to patients on first come first serve basis duly shown on the monitor screen. For each type of services, the patient has to register and obtain a number coupon and the patients shall be attended based on the number system.
